I came across this last weekend at the AUXCOMM course, and thought I would share what CT DEMHS expects:

Connecticut Interoperability Field Operations Guide - Page i

Common Responsibilities of All Deployed Responders
* Be prepared to deploy – have GO kit(s) ready
* Receive assignment – DO NOT self-deploy
* Reporting location, time, and travel route
* Description of type and severity of incident
* Instructions for communication while enroute
* Monitor incident status while enroute
* Start an ICS214 to document activities
* Upon arrival, check in at correct location
* Receive briefing from immediate supervisor
* Gather information on current incident status
* Copy of IAP, face-to-face, ICS201
* Document current situation if necessary
* Document significant activities on ICS214
* Maintain asset accountability
* Brief, assign, and manage subordinates, if any
* Work safely – ensure safety
* Participate in planning process if appropriate
* Keep system operational – verify!
* Brief replacement prior to shift change
* Demobilize as directed – when and how

Form ICS 214: A standardized "Activity Log" sheet
- This is an improvement over the Student Composition Book that I use as a LOG BOOK for each event.

IAP - Incident Action Plan
THE PLAN created as part of the Incident Action Planning Cycle and put into use by the Incident Commander or Unified Command and presented at the Operations Briefing.

Form ICS 201: Incident Briefing Form

- Developed from the Incident Action Plan as presented in the Operations Briefing

I plan on making these forms (and others) available in the "Forms" section of "DOWNLOADS" in the next week.
